United Healthcare Medicare HMO Some non-Medicare United Healthcare plans allow out-of network benefits.  Call to verify your coverage.
OHIO MEDICAID  "DISABILITY ASSISTANCE"  CARD No benefit for routine eye exams or eyeglasses.

We do accept other Medicaid cards.  

MEDICARE SSI card does NOT allow ROUTINE EYE EXAM, but WILL cover examination and treatment of eye disease.  Call if you need clarification.
