RELEASE OF PRESCRIPTIONS & MEDICAL RECORDS

Management of Medical Records is in compliance with the Rules of the Ohio State Board of Optometry and the Ohio Revised Code 3701.741.

 

SPECTACLE PRESCRIPTION

Released for a period of up to 3 years from Examination Date.  Prescriptions over 1 year old are generally considered expired, and re-examination is recommended.  You may call or stop in to receive a copy of your prescription at any time.  We prefer you call a day ahead, to allow time to review your record and have the prescription ready for you.  There is no fee for this service.

Requests for your prescription from another Doctor's office will be honored, if the request is faxed to our office at [888] 631-1436, and contains complete name, address, phone and fax numbers of the provider, and clearly identifies the patient.  For accuracy, please print your name and birth date on the form, and your signature is required (alternatively, signature of the Parent or Guardian of Minor Child, or Guardian or POA of Adult patient).

MEDICAL (EXAMINATION) RECORDS

Your examination record is property of Suburban Vision Care, Inc.  You are entitled to a copy of your examination records, with a properly completed HIPPA compliant Release of Medical Records Form, which will be posted here soon, in PDF format for you to download.  In the interim, call our office, and we will mail you a copy.  There may be a fee associated with processing your request.
