Please bring the following with you the day of your examination:

List of all current medications and dosages.

List of all medication allergies.

Name/address of referring physician, if any.

Family medical history of eye and systemic disease.

Insurance information.

Previous eyeglasses and/or contact lenses.

"Designated driver."    We may dilate your pupils on the day of your examination, or we may perform this test on a subsequent visit. When your eyes are dilated, you may want to bring sunglasses and someone to drive you home.

Please allow 45 minutes for your examination, and extra time if you will need to choose eyeglasses or are being fit with contact lenses for the first time.


Your comprehensive eye examination will include:

Thorough eye and medical history.

Examination of all parts of the eye, with state-of-the-art equipment.

Computer-assisted refractive analysis for eyeglass prescriptions, if needed.

Visual fields, color vision, examination for eye signs of neurological or systemic disease.

Detection of amblyopia (lazy eye), eye muscle balance, depth perception. 

Testing for glaucoma, cataracts, macular degeneration, diabetic eye conditions, and other pathology.

Additional testing, as medically indicated, for the diagnosis and treatment of eye disease.
